Free Sparks and Taylor's Nursing Diagnosis Reference Manual 9th edition
This book was the recommended one for writing our care plans. It's not easy to align the goals to interventions. I'm ordering the Gulanick version and will use both to cross check.

A great reading guide for the concept, altough i havent used it myself since its a gift, i have seen much of what it offers and is deffinently worth it.
