I had the previous edition of this book and there were many sections where the explanation was not clear enough. Upon google searching, I discovered others were posting questions about the same passages on forums and asking for its clarification. This edition has resolved the ones I checked.
As others have stated, the pages total have increased and so has the number of images. Online you have access to the book as well and can download all the images of the book in one zip file. This book is almost perfect, except some minor mistakes with incorrect referencing of images and tables as if they weren't updated etc (I found 2 in the respo chapter so far). Also binding is a bit weak and it's cracking a bit from the inside, but that could be because I am using it so much.
Pro:
Numbers of pages increased.
More explanations/clarifications than before.
Online access to book (searchable), images, qbank, and errata.
Con:
A few minor typos.
Conclusion: This edition is a MUST, even if you had a previous one.

Huge improvement over the 3rd edition. 200 pages more than the 3rd edition, but more readable in my opinion, yes I bought the 3rd edition last year, and I had to shell out another $35 for this edition, but what is $35 to one of the most important Step 1 resources? I'm glad I got it.
